DECISION
On revoking certificate of eligibility for drug business

Pursuant to the Law on Pharmacy dated April 06, 2016;
Pursuant to the Decree No 54/2017/NĐ-CP dated May 08, 2017;
Pursuant to the Decree No 75/2017/NĐ-CP dated Jun 20, 2017;
Pursuant to the application dated Oct 17, 2017 of Mega Lifesciens (Vietnam) Ltd. about termination of drug storage service activities
At the request of Director of DAV,
DECIDE:
Article 1. Revoke 02 certificates of eligibility for drug business as below:
- Certificates of eligibility for drug business No 174/BYT-ĐKKĐ dated 11/7/2016 of Mega Lifesciens (Vietnam) Ltd.
– Office address: 364 Cong Hoa str., 13 ward, Tan Binh Dist., HCM city (E-town building).
– Chief Pharmacist: Tran Thanh Dat
– Kind of business: Business providing drug storage services
– Scope: Services of finished product storage at normal and cold temperature (not including vaccine, biologics, narcotic drugs, psychotropic drugs, drug precursors)
– Warehouse address: Unit IV-13, No. 2 str., IV industrial group, Tan Binh Industrial, Tay Thanh ward, Tan Phu Dist., HCM.

Certificates of eligibility for drug business No 3554/BYT-ĐKKĐ dated 19/09/2013 of Mega Lifesciens Ltd. In Hanoi
– Office address: 4A Lang Ha str., Thanh Cong ward, Ba Dinh Dist., Hanoi.
– Chief Pharmacist: Nguyen Duc Ha
– Kind of business: Business of drugs.
– Scope: Services of finished product storage.
– Warehouse address: Cuong Ngo, Tu Hiep, Thanh Tri, Hanoi.
Article 2. This Decision comes into force from the signing date.
Article 3. Chief of the office, Director of DAV, Director of Mega Lifesciens (Vietnam) Ltd., Director of Mega Lifesciens Ltd. in Hanoi, Director of department of health of Hanoi, Director of department of health of Hochiminh are responsible for the implementation of this Decision.
